The Carroll County Health Department is currently vaccinating people in phases 1a1, 1a2, 1b, and people 65 & older.

To schedule an appointment, you will need to go to https://covid19.tn.gov or call 1-866-442-5301.

 Baptist Memorial Hospital-Carroll County

    Baptist Memorial Hospital-Carroll County us now offering the COVID-19 vaccine to groups: 1a1, 1a2, and 1b and to those persons aged 65 & older.

    Baptist will be administering vaccines from 8:00 a.m. to 4:30 p.m. by appointment only.

Call weekdays 8:00 a.m. to 5 p.m. to schedule your appointment at 731-986-7400.

    *If you are showing symptoms of COVID-19, please wait until a later date to schedule your vaccination.

 1099-G Questions

    If you have Employer or Individual questions, here is about where to get help if:

1. They have received 1099-G statement regarding unemployment income, but never drew UI in 2020 or

2.  If there is a mistake on their 1099-G

The latest State UI Division helpline number that has been provided is (844)500-4906. Feel free to provide them with this information if they contact you for guidance.
